What Is an Appropriate PaCO2 for Patients with Severe TBI?

PaCO2 is one of the critical determinants of cerebral blood flow and hence brain volume. A reduction in PaCO2 decreases ICP and common practice is to aim for a PaCO2 of 4.5–5.0 kPa. Further decreases in PaCO2 continue to decrease brain volume, but at the expense of cerebral blood flow with below normal PaCO2 being associated with worse outcomes [15]. However, targeted short term hyperventilation may have a role on the management of refractory intra-cranial hypertension, this is covered in more detail in Part 2 of this chapter.
